Subject: Discharge Summary for Mrs. Mary Thompson
Dear Dr. Smith,
I am writing to inform you about the recent hospitalization and subsequent discharge of your patient, Mrs. Mary Thompson, a 68-year-old female with a history of hypertension and type 2 diabetes. Mrs. Thompson was admitted to our hospital on 15th July 2023 after suffering a fall at home, resulting in a right hip fracture.
Post her admission, a successful right hip hemiarthroplasty was performed on the 16th of July. Her post-operative blood tests indicated mild anaemia with a haemoglobin level of 110g/L, prompting the initiation of oral iron supplements. Mrs. Thompson subsequently developed a urinary tract infection, confirmed by urine culture. It was effectively managed with a seven-day course of oral Ciprofloxacin 500mg twice daily.
Despite beginning physiotherapy on the 18th of July and mobilizing with a frame, Mrs. Thompson reported high levels of anxiety about potential future falls. A psychiatrist was consulted and she was prescribed Lorazepam 0.5mg at night.
On the 19th of July, Mrs. Thompson experienced constipation, likely due to her change in mobility and diet. Lactulose syrup was administered to alleviate this issue.
Mrs. Thompson’s discharge is scheduled for tomorrow. Her current medications include Paracetamol 1g four times daily, Ferrous Sulphate 200mg once daily, Calcium and Vitamin D supplements, Amlodipine 5mg daily, Metformin 500mg twice daily, Lorazepam 0.5mg at night, and Lactulose syrup.
